Father’s Day questionnaires have always been one of my favorite end-of-the-year activities because students love sharing all the things that make their dads, grandpas, or special people so important to them. Their responses are often sweet, thoughtful, and sometimes downright hilarious!
Unfortunately, many traditional Father’s Day questionnaires can be difficult for young learners to complete independently.
They often include too many questions, require a lot of writing, and can quickly become overwhelming for preschool, kindergarten, and first grade students.
That’s why I created these Father’s Day questionnaires specifically for younger learners. I wanted something simple, meaningful, and fun that would allow students to share their thoughts without becoming frustrated by too much writing.
Designed for Preschool, Kindergarten, and First Grade
These Father’s Day questionnaires were created with young students in mind.
Instead of long lists of questions, each page includes:
- Short, kid-friendly prompts
- Plenty of space for writing
- A drawing or coloring area for students to personalize their work
- Fun themes that keep students engaged
Because the writing load is manageable, students are more likely to complete the activity independently and feel successful.
To make them even more flexible, both full-page and half-page versions were created. The full-page versions provide extra writing space for students who need it, while the half-page versions work well for quick activities or keepsake booklets.
Since every family looks different, versions are also included for grandpas, uncles, brothers, special people, and other important caregivers.
5 Fun Father’s Day Questionnaire Themes
I love giving students and teachers choice, so I created 5 themed questionnaires that could easily coordinate with different Father’s Day celebrations, gifts, or classroom activities.

Each theme includes simple prompts and a drawing or coloring component so students can create a personalized keepsake for Father’s Day.

The prompts are intentionally short and manageable, allowing students to focus on what really matters: sharing the things they love most about their dad or special person.
